Successful Homecare Agency Silver Package Requirements
Need Own CNA/PCA and as Caregiver in addition to RN
This package includes me personally making sure your Private Home Care Provider (PHCP) agency licensure process is simple and stress-free. I will properly complete all Georgia state required paperwork for your PHCP licensure application. This will also include the policies and procedures customized to your agency. I will provide the required in office CPR, First Aid and AED training for your caregiver staff. In addition, your caregiver staff will receive their signed required certificate approved by the American Heart Association that allows your CNA/PCA to safely perform their duties in a client’s home. Also included is a state required mandatory background check application up to three employees for your organization. When you select the silver package, if a notary is needed, we will provide that too. We will correctly handle all the details so you can focus on building your successful homecare agency with confidence.
Successful Homecare Agency Gold Package Requirements
RN and CNA/PCA for Caregiver is Provided
- The Successful Homecare Agency Gold Package includes everything offered in the Silver Package as I have listed below. PLUS documentations of a licensed Registered Nurse in the state of Georgia. I will also provide you a Certified Nurse Aide or a Personal Care Attendant. This includes all documentations required for your PHCP licensure application. As I stated previously, here is everything else you will receive in the Gold Package.
- Step-by-step guidance through the PHCP licensure process to make it smooth and stress-free
- Completion of all required Georgia state paperwork for your PHCP application
- Custom policies and procedures tailored to your agency
- CPR, First Aid, and AED training for caregiver staff
- American Heart Association–approved certificates for CNAs/PCAs to safely perform client care
- Mandatory background check applications for up to three employees

Successful Homecare Agency Platinum Package Requirements
RN and CNA/PCA for Caregiver is Provided Preparation for medicaid license Provide survey ready business with charts with 2 patients
If you want the Successful Homecare Agency Platinum Package this package includes everything in the Silver & Gold Packages Plus 3 completed charts; 1 for the Registered Nurse, 1 for the Client, 1 for the staff. These charts can be used to onboard all future clients, caregivers and Registered Nurses/Licensed Practical Nurses. For the staff files, contents will include job application, Government ID, TB result, CPR certification, and other HR documents which includes Direct Deposit bank form. For client files, contents will include Nursing Assessment, Medication List, Physician List, Service Plan, Service Agreement, and other necessary Authorization forms. For the RN/LPN files, contents include job application, active RN /LPN License, Job Description, CPR Certification, TB result, Direct Deposit Bank forms, other HR documents. All services in Package 3 plus video training, website and preparation for GA HFR state survey.
